Upon completion of this form, we will endeavour to contact you within 24hours to confirm all details of the request and payment details prior to undertaking any requested service.

I hereby agree to Asset Plant & Machinery's charges and terms and conditions:
$195 + GST PER HOUR Labour - (1 hour minimum)
$195+ GST PER HOUR Travel - (1 hour minimum)
Materials and parts are not included in this price and will be charged accordingly.

Credit card details must be provided by any non-Asset Plant customers or non-credit account customers prior to commencement of service work – this is for security purposes – This will be done over the phone/email upon receipt of the submitted service request.

**IF NO DETAILS PROVIDED IT WILL CAUSE DELAYS IN YOUR REPAIRS**

Asset credit account customers (credit is provided only upon application) will be sent an invoice to be paid within agreed upon payment period upon works completion.

Other Asset Plant customers be sent an invoice to be paid within 7 days. If not paid within this period, the nominated card will be charged.

EFT payments to be made within 7 days of invoice – if payment not received we reserve the right to deduct from the supplied credit card.

FOR WARRANTY CLAIMS - I acknowledge that if Asset Plant & Machinery service agent inspects and reports on my machine and deems the fault to be caused by incorrect installation and/ or misused I will be charged by Asset Plant & Machinery at above rates as per our Terms and conditions. No non authorised warranty repairs will be paid by Asset Plant unless previously approved. If approval is given to Asset Plant customer to arrange repairs to a machine under warranty that Asset cannot attend, quote must be provided for any repairs prior to commencement if amount exceeds $500 - otherwise this will not be accepted.
